Step 4: Write (CCM Notes or BHI or RPM Notes with a colon) LIKE "CCM Notes:" and Copy notes and task in to GURU
Step 5: Read and Fill in the blanks and paste notes in Medimote.
Step 6: Do not included "CPT-aligned Phrasing for Billing Audits:" and any information that is not needed.

Spoke with Shelly Smith for an in-depth care management conversation. Shelly warmly consented to the chronic care management program and expressed strong loyalty to Dr. HAPPY, sharing that she was extremely stressed during her recent insurance transition. She made it clear that she would have continued seeing Dr. HAPPY even if it required paying out-of-pocket, as she feels he has walked with her through a critical chapter of her life—recovering from methadone addiction. Shelly detailed that it took five years to fully heal and transition off methadone, a process she completed during the COVID-19 pandemic. Reflecting on her recovery, she said “there were few sunny days, and then more and more sunny days came,” symbolizing her emotional and physical progress. She shared that she is currently taking Adderall, originally prescribed during her recovery process. She expressed concern about the long-term use and the possibility of developing dependency. Shelly admitted she’s hesitant to bring it up to Dr. Happy due to fear and uncertainty but wants to collaborate on a long-term tapering plan. While she has noticed minor irritability, she reported no cardiac side effects. She’s particularly interested in exploring natural alternatives and mentioned she avoids searching for health information online due to overwhelm. Shelly also provided insight into her personal background: She is African American and the youngest of three sisters. Her full name is Shelly Diane Smith. She currently works for the Ford Catalog Department—her third time working for the company. She noted Ford’s catalog technology is outdated, which creates daily operational challenges, but she is proud of her ability to work through them.
